ABSTRACT

Resumen Introducción: Las encefalitis inmunomediadas son un desorden neurológico de origen autoinmune. Actual mente es escasa la descripción de las secuelas cognitivas crónicas. El objetivo del presente trabajo fue caracterizar la secuela cognitiva de diferentes tipos de encefalitis inmunomediadas en una cohorte de un centro único de Argentina. Métodos: Estudio prospectivo, observacional, trans versal, de pacientes en seguimiento en un hospital de la Ciudad de Buenos Aires, con diagnóstico de encefalitis inmunomediada probable y definitiva. Se evaluaron variables epidemiológicas, clínicas, paraclínicas y tra tamiento. Se determinó la secuela cognitiva a través de una evaluación neurocognitiva realizada a partir del año de la presentación clínica. Resultados: Fueron incluidos 15 pacientes, todos con resultado disminuido en al menos un test. La memoria fue el dominio más afectado. Aquellos que se encon traban bajo tratamiento inmunosupresor al momento de evaluarse presentaron menores resultados en el aprendizaje seriado (media -2.94; desvío estándar 1.54) versus los que se encontraban sin tratamiento (media -1.18; desvío estándar 1.40; p = 0.05) y en la prueba de reconocimiento (media -10.34; desvío estándar 8.02) ver sus sin tratamiento (media -1.39; desvío estándar 2.21; p = 0.003). Los pacientes con estatus epiléptico tuvieron resultados deficitarios en la prueba de reconocimiento (media -7.2; desvío estándar 7.91) en comparación a los que no lo tenían (media -1.47; desvío estándar 2.34; p = 0.05). Conclusión: Nuestros resultados demuestran que, a pesar del curso monofásico de la enfermedad, todos los pacientes presentan daño cognitivo persistente más allá del año del inicio del cuadro. Estudios prospectivos de mayor envergadura serían necesarios para confirmar nuestros hallazgos.


Abstract Introduction: Autoimmune encephalitis represents a group of immune-mediated neurological disorders. At present, the description of the chronic cognitive sequela is scarce. The objective of this study was to characterize the cognitive after effects of different types of autoimmune encephalitis in a cohort from a single center in Argentina. Methods: Prospective, observational, cross-sectional study of patients under follow-up at a hospital in Buenos Aires city, with a diagnosis of probable and definitive immune-mediated encephalitis. Epidemiological, clini cal, paraclinical and treatment related variables were evaluated. Cognitive sequela was determined through a neurocognitive evaluation performed at least a year after the clinical presentation. Results: Fifteen patients were included. All had di minished results in at least one test. Memory was the most affected domain. Patients who were under im munosuppressive treatment at the time of evaluation presented lower results in serial learning (mean -2.94; standard deviation 1.54) versus those who weren't under treatment (mean -1.18; standard deviation 1.40; p = 0.05). The same pattern was observed on the recognition test of treatment group (mean -10.34; standard deviation 8.02) versus treatment-free group (mean -1.39; standard deviation 2.21; p =0.003). Patients with status epilepticus had poorer results in the recognition test (mean -7.2; standard deviation 7.91) compared to those without it (mean -1.47; standard deviation 2.34; p = 0.05). Conclusion: Our results show that, despite the mo nophasic course of this disease, all patients had persis tent cognitive damage beyond the year of onset. Larger prospective studies are required to confirm our findings.

ABSTRACT

Objective:To explore the application of Tongdu Tiaoshen acupuncture in patients with hemiplegia after stroke.Methods:Randomized controlled trial. A total of 70 patients who met the inclusion criteria for post-stroke hemiplegia in our hospital from January 2020 to December 2021 were divided into two groups, with 35 cases in each group, according to the random number table. The control group was given conventional treatment combined with rehabilitation exercise therapy, and the observation group was given Tongdu Tiaoshen acupuncture on the basis of the above, and both groups were treated continuously for 1 month. Compare of the two groups Fugl-Meyer Assessment (FMA), Modified Ashworth Scale (MAS), National Institutes of Health Stroke Scale (NIHSS), Activity of Daily Living (ADL) score; Brunnstrom stage method was used to assess the patient's motor ability, and Holden walking function grade was used to assess the patient's walking ability. The activated partial thromboplastin time (APTT), prothrombin time (PT), thrombin time(TT), and serum fibrinogen (FIB), D-dimer (D-D), plasminogen activator inhibitor 1 (PAI-1) levels were observed by ELISA. Adverse events were recorded and clinical efficacy was evaluated.Results:The total effective rates of the observation group and the control group were 97.14% (34/35) and 77.14% (27/35), respectively, and the difference between the two groups was statistically significant ( χ2=6.25, P=0.012). After treatment, the scores of FMA and ADL in the observation group were significantly higher than those in the control group ( t values were 9.23 and 9.54, respectively, P<0.01), the MAS and NIHSS scores were significantly lower than those in the control group ( t values were 10.23 and 11.97, respectively, P<0.01). After treatment, the distribution of patients in Brunnstrom stage Ⅴ and Ⅵand Holden functional walking stage Ⅳ and Ⅴ in the observation group was significantly higher than those in the control group ( χ2 values were 11.96 and 11.27, respectively, P<0.05). After treatment, APTT, PT and TT in the observation group were significantly longer than those in the control group ( t values were 10.37, 13.57 and 6.54, respectively, P<0.01); serum FIB, D-D and PAI-1 levels were significantly lower than those in the control group ( t values were 12.85, 11.94 and 27.39, respectively, P<0.01). No adverse reactions occurred in both groups. Conclusion:The treatment of post-stroke hemiplegia with Tongdu Tiaoshen acupuncture can improve the effect of clinical treatment and the ability of daily life, neurological function, limb function, movement and walking ability, and stabilize the clotting state.

ABSTRACT

Objective:To explore the effect of Qixian Tongluo Prescription fumigation on nerve function and rehabilitation effect in patients with hemiplegia after cerebral infarction of qi-deficiency blood stasis syndrome.Methods:Randomized controlled trial. Sixty-eight patients with hemiplegia after cerebral infarction of qi-deficiency blood stasis syndrome in the hospital were enrolled between October 2020 and December 2021. According to random odd-even numbering method, participants were divided into the control group (routine western medicine) and the observation group (Qixian Tongluo Prescription fumigation on basis of control group), 34 in each group. All were continuously treated for 8 weeks. TCM syndromes were scored before and after treatment. The severity of neurological impairment was evaluated by National Institutes of Health Stroke Scale (NIHSS). The severity of limb movement disorder was evaluated by Fugl-Meyer Assessment (FMA). The levels of central nervous specific protein (S-100β), neuron-specific enolase (NSE), endothelin (ET), nitric oxide (NO), C-reactive protein (CRP) and superoxide dismutase (SOD) were detected by ELISA. The clinical response rate was assessed.Results:There were significant differences in total response rate between the observation group and the control group [94.12% (32/34) vs. 76.47% (26/34); χ2=4.22, P=0.040]. After treatment, scores of hemiplegia, shortness of breath, palpitation and limbs swelling in observation group were significantly lower than those in the control group ( t=3.44, 2.37, 2.72, 3.89, P<0.05 or P<0.01), NIHSS score was significantly lower than that of the control group ( t=6.56, P<0.01), and FMA scores of upper and lower limbs were significantly higher than those in the control group ( t=2.17, 2.78, P<0.05). After treatment, levels of serum S-100β [(0.69±0.27) μg/L vs. (0.85±0.36) μg/L, t=2.07], NSE [(8.36±3.69) μg/L vs. (11.34±4.93) μg/L, t=2.82] and ET [(53.16±12.12) ng/L vs. (61.25±11.31) ng/L, t=2.85] in observation group were significantly lower than those in the control group ( P<0.05 or P<0.01), while NO [(82.26±14.53) μmol/L vs. (70.16±12.27) μmol/L, t=3.71] was significantly higher than that of the observation group ( P<0.01). The level of serum CRP in observation group was significantly lower than that of the control group ( t=2.74, P<0.01), and SOD activity was significantly higher than that of the control group ( t=2.49, P<0.05). Conclusion:Qixian Tongluo Prescription fumigation can promote the recovery of nerve function and vascular endothelial function in patients with hemiplegia after cerebral infarction, improve limb disorders and clinical effect.

ABSTRACT

Objective:To evaluate the effect of scalp acupuncture combined with repeated transcranial magnetic stimulation (rTMS) on dysphagia after ischemic stroke.Methods:Prospective cohort study. A total of 94 patients with dysphagia after ischemic stroke in our hospital who met the inclusion criteria from December 2020 to February 2022 were randomly divided into two groups with 47 patients in each group. The control group was treated with rTMS and conventional acupuncture, while the scalp acupuncture group was treated with rTMS and scalp acupuncture (scalp acupuncture+seven points at the skull base). Both groups were treated for 4 weeks. Before and after treatment, Traditional Chinese Medicine (TCM) symptoms were scored, and the morphology, movement and food swallowing process of the patient's swallowing anatomy were observed with fiber optic endoscopy (FEES), and assessed by the leakage aspiration scale (PAS). The Yilang Fujishima swallowing efficacy evaluation and standard swallowing assessment (SSA) were used to evaluate swallowing function. The dysphagia quality of life scale (SWAL-QOL) were used to evaluate patients' quality of life. Magnetic resonance diffusion tensor imaging (DTI) scanning was performed to observe the anisotropy fraction (FA value) of the selected region of interest (ROI).Results:During the treatment period, each group dropped off 1 patient, and 92 patients entered the statistics. After treatment, the scores of salivation, food retention, eating and drinking cough in the scalp acupuncture group were significantly lower than those in the control group ( t values were 6.87, 4.90, 5.01, respectively, P<0.01), and the PAS grading was significantly better than that in the control group ( χ2=7.80, P=0.025), the swallowing efficacy evaluation and SWAL-QOL score of Yilang Fujishima were significantly higher than those of the control group ( t=6.81, 5.98, P<0.01), and the SSA score was significantly lower than that of the control group ( t=5.68, P<0.01). The FA values of parieto occipital cortex (0.47 ± 0.06 vs. 0.42 ± 0.04, t=3.16), insular cortex (0.44 ± 0.07 vs. 0.40 ± 0.05, t=3.00) and posterior limb of internal capsule (0.58 ± 0.04 vs. 0.54 ± 0.05, t=2.80) of ROI in the head acupuncture group after treatment were significantly higher than those in the control group ( P<0.05). Conclusion:The scalp acupuncture combined with rTMS can improve the swallowing function of patients with dysphagia after ischemic stroke and improve their quality of life.

ABSTRACT

Introducción: El espectro de secuelas de la tortícolis muscular congénita es amplio. Algunos niños con dichas secuelas requieren de costosas y complejas operaciones. Objetivo: Determinar los factores precondicionantes asociados a secuelas orgánicas de la tortícolis muscular congénita en pacientes desde un mes hasta 5 años de edad. Métodos: Se realizó un estudio retrospectivo, analítico, no experimental de casos y controles que incluyó a niños con diagnóstico de tortícolis muscular congénita, atendidos en la consulta de Neurodesarrollo del Hospital Infantil Antonio María Beguez César de Santiago de Cuba, desde enero del 2017 hasta diciembre del 2020. El grupo de casos quedó conformado por todos los pacientes con complicaciones orgánicas de la tortícolis muscular congénita (24 niños) y el de control, por quienes no presentaron dichas complicaciones (72), seleccionados mediante un muestreo aleatorio simple. Resultados: El sexo masculino predominó en los pacientes con complicaciones (62,5 %). La plagiocefalia y la asimetría facial fueron las secuelas más frecuentes. La edad menor de 6 meses en el momento del diagnóstico de la entidad constituyó un factor protector (OR = 0,12; IC 95 %: 0,04-0,33; p=0,000); hubo asociación de las complicaciones con el parto distócico (OR = 0,36; IC 95 %: 0,13-1,01; p=0,049) y con la macrosomía fetal (OR = 5,7; IC 95 %: 1,44-22,2; p=0,014). También existió asociación altamente significativa de las complicaciones con la asimetría de ambos músculos esternocleidomastoideos (OR = 11,4; IC 95 %: 3,7-34,9; p=0,000) y con el inicio tardío del tratamiento (OR = 8,5; IC 95 %: 3,0-24,1; p=0,000). Conclusiones: El parto distócico, la macrosomía fetal, la asimetría entre ambos músculos esternocleidomastoideos y el inicio tardío del tratamiento rehabilitador incrementaron el riesgo de complicaciones en los integrantes de la casuística.


Introduction: The spectrum of sequela of the congenital muscular wryneck is wide. Some children with these sequela require expensive and complex operations. Objective: To determine the precondicionant factors associated with organic sequela of the congenital muscular wryneck in patients since one month to 5 years. Methods: A retrospective, analytic, non experimental of cases and controls study was carried out that included children with diagnosis of congenital muscular wryneck, assisted in the Neurodevelopment Service of Antonio María Beguez Cesar Children Hospital in Santiago de Cuba, from January, 2017 to December, 2020. The group of cases was conformed by all the patients with organic complications of the congenital muscular wryneck (24 children) and the control group, for those who didn't present these complications (72), selected by means of a simple random sampling. Results: The male sex prevailed in the patients with complications (62.5 %). The plagiocephaly and the facial asymmetry were the most frequent sequela. The age under 6 months in the moment of the diagnosis of the entity constituted a protective factor (OR = 0.12; IC 95 %: 0.04-0.33; p=0.000); there was association of the complications with the dystocic childbirth (OR = 0.36; IC 95 %: 0.13-1.01; p=0.049) and with the fetal macrosomia (OR = 5.7; IC 95 %: 1.44-22,2; p=0.014). There was also highly significant association of the complications with the asymmetry of both sternocleidomastoid muscles (OR = 11.4; IC 95 %: 3.7-34.9; p=0-000) and with the late beginning of the treatment (OR = 8.5; IC 95 %: 3.0-24.1; p=0.000). Conclusions: The dystocic childbirth, fetal macrosomia, asymmetry between both sternocleidomastoid muscles and the late beginning of the rehabilitative treatment increased the risk of complications in the members of the case material.

ABSTRACT

Objective:To explore the curative effect of Buyang Huanwu Decoction combined with suspension exercise training for the patients with cerebral infarction combined and lower limb hemiplegia.Methods:According to random number table method, 94 patients with cerebral infarction and lower limb hemiplegia meeting the inclusion criteria were divided into the control group and the observation group between January 2017 and February 2021, 47 in each group. The control group was treated with suspension exercise training, while observation group was additionally treated with Buyang Huanwu Decoction on the basis of the control group treatment. All were treated for 6 weeks and then followed up for 3 months. Before and after treatment, the Traditional Chinese Medicine (TCM) syndromes scores were recorded. The severity of nerve function injury was evaluated by National Institutes of Health Stroke Scale (NIHSS). The lower limb function was evaluated by Fugl-Meyer Assessment (FMA). The balance function was evaluated by Berg Balance Scale (BBS). The activities of daily life were assessed by Barthel Index (BI). The whole blood high shear viscosity, plasma viscosity and hematocrit were detected by full-automatic hemorheology analyzer. The levels of plasma total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) were detected by full-automatic analyzer. All were followed up for 3 months. The prognosis of patients was assessed by modified Rankin scale (mRS). The adverse reactions during treatment were recorded.Results:After treatment, scores of TCM syndromes and NIHSS in observation group were significantly lower than those in control group ( t=5.35, 4.54, P<0.01), while scores of FMA, BBS and BI were significantly higher than those in control group ( t=3.40, 3.10, 7.57, P<0.01). The whole blood high-shear viscosity, plasma viscosity and hematocrit in observation group were significantly lower than those in control group ( t=2.94, 3.81, 4.23, P<0.05 or P<0.01), and levels of TC, TG and LDL-C were significantly lower than those in control group ( t=4.10, 4.27, 3.61, P<0.01). The differences in good prognosis rate between observation group and control group were statistically significant [74.47% (35/47) vs. 51.06% (24/47); χ2=5.51, P=0.019]. Conclusion:The Buyang Huanwu Decoction combined with suspension exercise training can relieve clinical symptoms, recover lower limb function, improve activities of daily life and prognosis in cerebral infarction combined with lower limb hemiplegia.

OBJECTIVE@#To observe the effect of thumb-tack needles based on "Biaoben acupoint compatibility" on sequela symptoms, mental state and pulmonary ventilation function in patients with coronavirus disease 2019 (COVID-19) during recovery period.@*METHODS@#Fifty cases of COVID-19 during recovery period were randomly divided into an observation group and a control group, 25 cases in each group. The patients in the observation group were treated with thumb-tack needles at Guanyuan (CV 4), Zusanli (ST 36) and Taiyuan (LU 9). The patients in the control group were treated with sham thumb-tack needles at identical acupoints as the observation group. The treatment in the two groups was given once a day, 7-day treatment was taken as a course of treatment, and totally two courses of treatment were given. The TCM symptom score, Hamilton anxiety scale (HAMA) score, Hamilton depression scale (HAMD) score, pulmonary function (forced vital capacity [FVC], forced expiratory volume in the first second [FEV1], peak expiratory flow [PEF]), the severity of pulmonary ventilation dysfunction and pulmonary imaging changes in the two groups were compared before and after treatment.@*RESULTS@#Compared before treatment, the total scores and each item scores of TCM symptom scale, HAMA scores and HAMD scores in the two groups were reduced after treatment (P<0.05). Except for the symptom scores of dry throat and dry stool, the total score and each item score of TCM symptom scale, HAMA score and HAMD score in the observation group were lower than those in the control group (P<0.05). Compared before treatment, FVC, FEV1 and PEF in the two groups were increased after treatment (P<0.05), and those in the observation group were higher than the control group (P<0.05). The severity of pulmonary ventilation dysfunction in the two groups was reduced after treatment (P<0.05), and the severity in the observation group was better than that in the control group (P<0.05). After treatment, the lung shadow area in the two groups was decreased (P<0.05), and that in the observation group was smaller than the control group (P<0.05). The improvement of imaging change in the observation group was better than that in the control group (P<0.05).@*CONCLUSION@#The thumb-tack needles based on "Biaoben acupoint compatibility" could significantly reduce the sequela symptoms, anxiety and depression in patients with COVID-19 during recovery stage, and improve the pulmonary ventilation function.

Objective: Traumatic dental injuries (TDIs) affect the aesthetics and function of primary teeth and can cause sequelae in their permanent successors. This study assessed the distribution of TDIs and sequelae in the primary dentition and the respective permanent dentition in children treated at the Dental Trauma Centre in Brazil. Material and Methods: This cross-sectional study was conducted by collecting data from 256 dental records over 7 years. Descriptive statistical analyses and the Fisher exact test were used (p=0.05). Results: Sixty-two patients presented with trauma in the deciduous teeth. TDIs were observed mostly in boys (64.50%) and in the 2 to 4-year age group (44.10%); most injuries occurred from fall from the child's own height (40.30%) and at home (71.20%). The most common type of trauma was subluxation (22.90%). The most prevalent clinical and radiographic sequelae were mobility (45.60%) and periapical lesion (31.80%), respectively. Regarding the permanent dentition, the most frequent sequela was hypomineralization (33.30%). Conclusion:According to this study, TDIs in deciduous teeth made up 29.24% of all the cases in the Dental Trauma Program. The major type of trauma in children was subluxation, with sequelae in both dentitions. Follow-up after trauma is important to the maintenance of primary dentition and to prevent and treat possible sequelae in the permanent dentition.(AU)


Objetivo: Lesões dentárias traumáticas (TDIs) afetam a estética e a função dos dentes decíduos e podem causar sequelas em sucessores permanentes. Este estudo avaliou a distribuição das TDIs e sequelas na dentição decídua e sua respectiva dentição permanente em crianças atendidas no Centro de Trauma Odontológico no Brasil. Material e Métodos: Trata-se de um estudo transversal realizado por meio da coleta de dados de 256 prontuários odontológicos ao longo de 7 anos. Foram utilizadas análises estatísticas descritivas e teste exato de Fisher (p <0,05). Resultados:Sessenta e dois pacientes apresentaram trauma na dentição decídua. As TDIs foram observadas principalmente em meninos (64,50%) e na faixa etária de 2 a 4 anos (44,10%); a maioria das lesões ocorreu por queda da própria altura (40,30%) e em casa (71,20%). O tipo de trauma mais comum foi a subluxação (22,90%). As sequelas clínicas e radiográficas mais prevalentes foram mobilidade (45,60%) e lesão periapical (31,80%), respectivamente. Em relação à dentição permanente, a sequela mais frequente foi a hipomineralização (33,30%). Conclusão: De acordo com este estudo, as TDIs em dentes decíduos foram de 29,24% de todos os casos do Programa de Trauma Dental. O principal tipo de trauma em crianças foi a subluxação, com sequelas em ambas as dentições. O acompanhamento após o trauma é importante para a manutenção da dentição decídua e para prevenir e tratar possíveis sequelas na dentição permanente. (AU)

ABSTRACT

Objective:To observe the clinical effect of internal administration of Traditional Chinese Medicine (TCM) and external application of hot election bag combined with acupuncture on urinary retention after stroke with kidney qi deficiency type.Methods:A total of 106 patients admitted to Chengde Hospital of Traditional Chinese Medicine from January 2017 to December 2020 who met the inclusion criteria were randomly divided into 2 groups according to the random number table method, with 53 in each group. The control group was treated with conventional western medicine therapy and bladder function training, while the observation group was treated with TCM, acupuncture and external application on the basis of the control group. Both groups were treated for 28 days. Before and after treatment, TCM syndrome scores were performed, and the maximum urinary capacity and residual urine volume were recorded by abdominal B-ultrasound to evaluate the bladder function of the patients. The improvement time of urinary pain, first urination time, catheter indwelling time, length of hospital stay and adverse reactions during treatment were observed and recorded, and the clinical efficacy was evaluated.Results:The total effective rate was 96.2% (51/53) in the observation group and 84.9% (45/53) in the control group, and the difference between the two groups was statistically significant ( χ2=3.98, P=0.046). The residual urine volume of the observation group after treatment [(54.23±6.23) ml vs. (91.24±11.25) ml, t=20.95] was significantly lower than that of the control group ( P<0.01), and the maximum urinary bladder volume [(366.23±30.23) ml vs. (259.63±26.23) ml, t=19.39] was significantly higher than that of the control group ( P<0.01). After treatment, the TCM syndrome score of the observation group was significantly lower than that of the control group ( t=13.25, P<0.01), and the bladder function score of the observation group was significantly lower than that of the control group ( t=13.53, P<0.01). The improvement time of urinary pain, first urination time, catheter indwelling time and hospital stay in the observation group were significantly lower than those in the control group ( t=5.73, 17.91, 6.76, 9.67, all Ps <0.01). No adverse reactions occurred in the two groups during treatment. Conclusion:The combination of TCM, hot compress therapy and acupuncture plus routine therapy can treat the patients with urinary retention after stroke and kidney qi deficiency type with good bladder function, improved symptoms and fast recovery and safety.

ABSTRACT

Objective:To observe the effect of self-designed Buqi Huoxue Decoction on the rehabilitation of patients with stroke sequelae and qi deficiency and blood stasis syndrome.Methods:A total of 116 patients with stroke sequelae treated from October 2019 to October 2020 were selected and divided into observation group and the control group by randomized digital tables, 58 patients in each group. The control group was given the conventional treatment, and the observation group combined self-designed Buqi Huoxue Decoction and conventional treatment. Both groups were treated for 3 months. The soluble vascular cell adhesion molecule-1 (sVCAM-1), nito-oxide (NO), endothelin (ET-1), endothelial-derived hyperpolarization factor (EDHF), insulin-like growth factor (IGF-II), interleukin-1β (IL-1β), interleukin-6 (IL-6) levels were detected by ELISA. The National Institutes of Health Stroke Scale (NIHSS) was used to evaluate the degree of neurological deficit, motor function was evaluated by the Fugl-Meyer motor function scale (FMA), patients' daily living ability was assessed by daily living ability assessment table (ADL), the QLQ-C30 was used to evaluate the QoL of patients. The TCM syndrome scores were evaluated. The clinical response was compared between the two groups.Results:The total response rate was 94.8%(55/58) in the observation group and 81.0%(47/58) in the control group. There was significant difference between the two groups ( χ2=3.98, P=0.046). After treatment, the scores of TCM syndrome and NIHSS of pruritus in the observation group were significantly lower than those in the control group ( t values were 5.61, 5.21, respectively, all Ps<0.01). After treatment, the scores of FMA, ADL QLQ-C30 of pruritus in the observation group were significantly higher than those in the control group ( t values were 3.61, 3.13, 9.38, respectively, all Ps<0.01). After treatment, the NO[(87.61±19.18) μmol/L vs. (77.93±17.26) μmol/L, t=2.81], EDHF [(21.14±6.14) μmol/L vs. (16.61±4.52) μmol/L, t=4.45] levels in the observation group were significantly higher than those in the control group ( P<0.01). After treatment, the sVCAM-1[(309.58±30.11) μg/L vs. (354.16±33.04) μg/L, t=7.46], ET-1 [(50.07±18.23)ng/L vs. (66.31±17.89)ng/L, t=4.76] levels in the observation group were significantly lower than those in the control group ( P<0.01). After treatment, IL-1β, IGF-Ⅱ, IL-6 levels in the observation group were significantly lower than those in the control group ( t values were 4.66, 8.14 7.43, respectively, all Ps<0.01). Conclusion:The treatment of self-designed Buqi Huoxue Decoction can improve the vascular endothelial function, inhibit the inflammation, reduce the nerve function damage, improve the limb movement function, daily life ability, quality of life and the clinical symptoms.

ABSTRACT

Tongue acupuncture therapy is an external treatment method of "micro acupuncture system" based on the theory of Zang Fu meridians and holographic theory. It acts on the tongue and can be used for the treatment of swallowing disorder after stroke. It has the advantages of improving tongue and pharyngeal function, high safety, simplicity and quickness. It is often used in combination with rehabilitation training, Traditional Chinese Medicine therapy or other acupuncture methods in clinic, and has achieved good curative effects.

ABSTRACT

Objective:To explore the influence of acupoint catgut embedding combined with exercise therapy on the rehabilitation effect of patients with upper limb spasticity after stroke and qi deficiency and blood stasis type.Methods:A total of 76 patients with qi deficiency and blood stasis type of upper limb spasticity after stroke who met the inclusion criteria in the hospital were enrolled between May 2018 and May 2020, and they were divided into two groups according to the random number table method, with 38 in each group. The control group was given conventional rehabilitation treatment, while the observation group was combined with acupoint catgut embedding and exercise therapy intervention on the basis of the control group. The degree of elbow spasm was assessed by Modified Ashworth Scale and hemiplegic hand function was evaluated by Hemiplegic Hand Function Classification Scale. Simplified Fugl-Meyer Scale was used to evaluate the limb motor ability and Modified Barthel Index was used for ability of daily living, and the adverse events during treatment were recorded.Results:After treatment, the improvement degree of elbow spasm in observation group was significantly better than that in control group ( Z=2.29, P=0.022), and the hemiplegic hand function classification was significantly better than that in control group ( Z=2.08, P=0.038). After treatment, the scores of upper and lower extremities and total score of Fugl-Meyer Scale in observation group were significantly higher than those in control group ( t=7.17, 7.40, 11.39, all Ps<0.001). While the scores of hemiplegia, faintness, shortness of breath and sallow complexion in observation group were significantly lower than those in control group ( t=7.82, 12.87, 12.55, 7.95, all Ps<0.001). The Barthel index in observation group was significantly higher than that in control group at 6 and 12 months of follow-up ( t=4.82. 6.45, all Ps<0.001). Conclusion:Acupoint catgut embedding combined with exercise therapy for intervention can improve the hemiplegic hand function, and enhance the limb motor function and daily living ability of patients with qi deficiency and blood stasis type of upper limb spasticity after stroke.

ABSTRACT

In ischemic stroke sequela phase, Rehmanniae Radix Praeparata-Corni Fructus drug pair has the effect in protecting damaged neurons, but its mechanism has not been clear. In this study, network pharmacology was used to predict the mechanism of Rehmanniae Radix Praeparata-Corni Fructus in the treatment of ischemic stroke sequela. Through database search and literature retrie-val, 40 active ingredients of Rehmanniae Radix Praeparata and Corni Fructus were obtained, and their targets were obtained through STITCH and TCMSP databases. The targets of ischemic stroke sequela were obtained through OMIM,GAD,TTD and DrugBank databases. By screening the intersections of active ingredients targets and stroke treatment targets, 21 potential targets were obtained. The DAVID database was used for GO enrichment analysis and KEGG pathway analysis of potential targets. GO enrichment analysis showed that Rehmanniae Radix Praeparata-Corni Fructus were mainly involved in regulation of blood pressure, negative regulation of extrinsic apoptotic signaling and positive regulation of angiogenesis. KEGG pathway analysis showed that Rehmanniae Radix Praeparata-Corni Fructus could inhibit inflammatory response and apoptosis signaling pathway by regulating HIF-VEGFA signaling pathway in neural stem cell proliferation, TNF signaling pathway and NF-kappaB signaling pathway. Molecular docking technique was used to verify that Rehmanniae Radix Praeparata-Corni Fructus component has a good binding activity with potential targets. The results showed that in ischemic stroke sequela phase, Rehmanniae Radix Praeparata-Corni Fructus drug pair could play an important role in recovering neural function, promoting the proliferation of neural stem cells, angiogenesis, preventing neural cells apoptosis and regulating inflammatory factors.

ABSTRACT

Fangji Dihuang Decoction and Fengyin Decoction, which were firstly recorded in Synopsis of the Golden Chamber, could be used to treat stroke. However, both of these two decoctions are used infrequently. There is a clear pathogenesis of modern pathophysiology behind Fangji Dihuang Decoction syndrome and Fengyin Decoction syndrome, which can be interpreted by modern language of science. Fangji Dihuang Decoction can nourish Yin, tonify kidney, extinguish the wind, and dredge collaterals. It could be used to treat mania caused by cerebral infarction, senile dementia, Alzheimer's disease and other cerebrovascular diseases, and opened up the precedent of nourishing Yin and expelling wind to treat stroke in later generations. Fengyin Decoction can clear heat, extinguish the wind, and calm the mind. It can be used to treat cerebral infarction, limb paralysis and epileptic seizures with heat syndrome. It may be equivalent to "oriental olanzapine", and opened up the precedent of smoothing liver and suppressing Yang to treat stroke in later generations. They can be used in the combination, with a high application value in treating stroke and mental diseases.

ABSTRACT

[Objective] Peripheral facial paralysis with sequelae (PFPS) reduces the quality of life (QOL) of patients. However, not much information is available on acupuncture treatment (AT) for PFPS. We report a case of a patient with PFPS whose sequelae and QOL were successfully improved by a combination of medication, physical therapy (PT) and AT.[Case] A 42-year-old woman. Her chief complaint was facial tightness, facial asymmetry, and synkinesis.[History of present illness] Left paralysis developed in November X-1, and Bell's palsy was diagnosed. Yanagihara Score (YS) was 6 points. She received intravenous steroid treatment during hospitalization for 9 days after second week after onset. In addition, she was given facial nerve decompression on January 24, X. but there wasno change. Subsequently, she visited the Department of Otolaryngology-Head and Neck Surgery, The University of Tokyo on 24 April. Electroneurography was 4%, YS was16 points and synkinesis was found. AT was started from May 29, X. [Acupuncture treatment] We performed AT on muscle for facial expression for contractural reduction. Also, weinstructed self-care techniques such as massage, open-eyelid exercise, individual muscle strengthening exercise and matters that require attention in everyday life. AT was performed 19 times, once every one to two weeks.[Assessment and Progress] Measurements of YS, synkinesis, synkinetic potential (SP), Facial Clinimetric Evaluation Scale (FS) were evaluated at about 7 and 15 months after onset. YS (point) changed from 24 to 34, synkinesis changed from (++) to (+), SP did not change, FS (point) increased from 41 to 57.[Discussion and Conclusion] Treatment for PFPS is limited and reduces QOL. We added regular acupuncture treatment to medication and PT for PFPS. As a result, we were able to relieve sequelae, contributing to improvement in the quality of life. It is suggested that AT might have efficacy for PFPS.

ABSTRACT

Impalement injuries, is a severe form of trauma, which are not common in civilian life. These injuries rarely occurs in major accidents. Abdomen, chest, limbs and perineum are often involved due to their large surface area. Thoracic impalement injury is usually a fatal injury, due to location of major vessels and heart in the thoracic cavity. These injuries are horrifying to site, but the patients who are lucky enough to make it to hospital, usually survive. Chances of survival are larger in right sided impalement injuries while central injuries are always died at the scene. Our patient, 25 years old male, was brought to the emergency room (ER) with large impaled metallic bar (about 2.5 feet long) in situ, in right sided chest. The patient was immediately shifted to operation room (OR) and was operated, his recovery was uneventful without any sequelae. Such patients should be treated and resuscitated according to advanced trauma life support (ATLS) protocols and operated without any delay for further investigations. Such operations are carried out by the most experienced surgeon team available. The impaled objects should not be processed if not necessary to avoid major hemorrhage and damage to vital structures, until the patient is in operation room. Large size and unusual position of impaled objects, makes the job difficult for surgeons/anesthetists. Although horrifying at scene, patients with thoracic impalement injuries are mostly young and healthy, and those who survive the pre-hospital phase are potentially manageable with proper resuscitation. Usually these patients make recovery without any further complications.

ABSTRACT

Objective To investigate the effect of therapeutic effects and recovery by serum thermal decomposition method in the treatment of elderly patients with sequela of cerebral infarction.Methods49 cases patients with cerebral infarction in our hospital were selected and randomly divided into the control group and the experiment group.23 cases in the control group were treated by conventional therapy.26 cases in the experiment group were treated on the base of the control group with serum thermal decomposition method.10 days a course of treatment, after 2 courses, the clinical efficacy and recovery of patients in the two groups were compared.ResultsThe total effective rate of the experimental group was 92.3% higher than that of the control group (65.2%), and the difference was statistically significant (P<0.05).Two groups of patients with neurological impairment score and the National Institutes of Health Stroke Scale (NIHSS) score were reduced.Compared with the control group, the neurological impairment score and NIHSS score of the experimental group were lower, the difference was statistically significant(P<0.05).The experiment group motor function (Fugl-Mayer) score was higher, the difference was statistically significant (P<0.05);the experiment group quality of life score was higher, the difference was statistically significant (P<0.05).ConclusionThe method of serum pyrolysis poisoning treatment of elderly patients with cerebral infarction sequelae can significantly reduce the degree of neurological impairment, improve patient quality of life and sequelae recovery.

ABSTRACT

Objective To observe the effect of electroacupuncture at the beginning and ending points of bicipital muscle on the superficial electromyography (sEMG) of the spastic limb in hemiplegia.Method Forty patients with spastic hemiplegia after cerebral stroke were divided by the random number table into a group of electroacupuncture at the beginning and ending points (group A) and a group of electroacupuncture at antagonistic muscles (group B). The former selected Ashi points at the beginning and ending points of bicipital muscle, while the latter selected points at the antagonistic muscles [Tianjing (TE10), Qinglingyuan (TE11), etc.], to receive perpendicular puncturing. The needles were retained for 30 min, and the sEMG of bicipital muscles in resting state was detected after the removal of the needles. The spastic bicipital muscle was examined by sEMG prior to the acupuncture treatment, and respectively after 2-week and 4-week acupuncture treatment, and the detected parameters included root mean square (RMS) and integrated electromyography (IEMG).ResultThe EMG and IEMG declined gradually in the two groups after the acupuncture treatment; the intra-group comparisons of the RMS and IEMG values at three time points, e.g. prior to acupuncture treatment, after 2-week acupuncture treatment and after 4-weekacupuncture treatment, showed that the values changed significantly compared to those at the previous time point (P0.05). The RMS and IEMG values presented same changing tendencies after 4-week acupuncture treatment in the two groups.ConclusionAcupuncture at the beginning and ending points and at the points on antagonistic muscles both can decrease the resting-state muscle tension in hemiplegia patients; sEMG is of certain significance in evaluating the treatment of hemiplegia.

[Objective]Discussion on clinical experience and academic ideas of Professor FU Weimin in treatment of viral encephalitis sequela.[ Method] From the etiology and pathogenesis, and the treatment method to expound the clinical experience and academic viewpoints of Prpfessor FU Weimin, summarize his law of characteristic medication and diagnosis thought,and put an example to exemplify that. [Results] Professor FU Weimin is good at treating this disease from phlegm and blood. He believes that the real standard based on six exogenous pathogenic factors or plague pathogen and other than evil not clear, long into the blood collaterals, phlegm heat and blood stasis obstruction, disturbing the brain orifices, are the symptoms of disease;and the virtual caused by the empty brains, heart and kidney failure and the losed senses and deities, are the causative organism. The pathological features are mainly composed of real or virtual inclusions.Treatment with the kaiqiao huatan tongyu methods are the basic principles, combined with tonifying the kidney and marrow, trafficing heart and kidney, which embodies the compatibility characteristics of eliminating pathogen without forgetting the acual. [Conclusion] Professor FU Weimin 's treatment of viral encephalitis sequela is effective, and can effectively control the disease, reduce the recurrence rate and improve patients ' self-care ability and quality of life.It's worthy of our study and promotion.

[Objective]To summarize the professor WU Lianzhong's experience in treatment of spasmodic torticollis and myelitis sequela by using acupuncture,so as to guide clinical practice better.[Methods] From following professor WU Lianzhong clinical studies,studying professor WU Lianzhong's medical cases,introducing Professor WU to treat spasmodic torticollis with Du channel four points and five points and treat myelitis sequela by using acupuncture and traditional Chinese medicine,we can summarize his academic thoughts and experience on treating spasmodic torticollis and myelitis sequela.[Results] Professor WU thinks the key pathogenesis of Du channel type spastic torticollis is Orifices blocked and consciousness delirium,so the treatment principle is to clear the heart and reduce the fire,and regulate consciousness,at the same time strengthen the Du channel and break the nodules,this method is effective in clinical treatment.He holds that the etiology and pathogenesis of myelitis mainly for lack of spirit,attack of exogenous pathogens,prolonged illness lead to deficiency of liver and kidney,qi and blood deficiency,all these lead to tendon and vessel malnutrition.The cardinal treatment principle is to nourish liver and kidney,tonify qi and blood,dredge the channels and collaterals,also comprehensivey treat this disease by acupuncture combined with traditional Chinese medicine,the clinical effect is satisfying.[Conclusion] Professor WU's experience in treatment of Du channel type spasmodic torticollis and myelitis sequela is worth studying and popularizing in clinic.
